AFFIDAVIT OF UNDERTAKING

We, SANNY A. BAJA, bondsman for the accused, of legal age, Filipino, married
and a resident of Brgy. Bongawisan, San Francisco, Southern Leyte and JOEL A. BAJA, of
Brgy. Bongawisan, San Francisco, Southern Leyte accused in NPS DOCKET No. VIII-07a-
INQ-24A-00013 entitled DENR/CENRO versus JOEL A. BAJA for VIOL. OF PD 705,
now pending before the Office of the Provincial Prosecutor, Sogod, Southern Leyte, after
having been sworn in accordance with law, depose and say THAT:
I, SANNY A. BAJA, bondsman, has posted a cash bond in the amount of TEN
THOUSAND PESOS (₱ 10,000.00) ONLY in Philippine Currency for each of the accused
as shown in Official Receipt Nos. _________________________________________,

1. This undertaking shall be effective upon approval and shall remain in force at all stages
of the case until its final determination, unless the proper Court directs otherwise;

3. Should I JOEL A. BAJA the accused herein, fail to appear at the trial without
