I, MARVIN DHEL CARAIG CALINGAY, of legal age, single, Filipino
citizen and with residence and postal address at 414 Calaylayan, Abucay,
Baatn, Philippines after having been duly sworn to in accordance with law
hereby depose and state that:
1. I am the natural father of ISAIAH EZEKIEL DE LA CRUZ who was
born on November 14, 2014 at Baguio City out of wedlock;
2. Said child was born out of my relationship with EDRELYN SECUATEN
DE LA CRUZ;
3. I hereby acknowledge that ISAIAH EZEKIEL DE LA CRUZ is my
TRUE and LAWFUL child for all legal intents and purposes with the
obligation of providing love, care, financial, spiritual and educational
support to my said minor child who is unfortunately born out of
wedlock;
4. I am executing this affidavit as the natural father and to attest to
the veracity and truthfulness of the foregoing and for all legal
intents and purposes it may serve.
