AFFIDAVIT

That I am PRESENTACION RAZONABLE SIATAN, of legal age, Filipino,

married and a resident of Lower Limonzo, Padada, Davao del Sur,
Philippines, after having been duly sworn to in accordance with the law
hereby depose and say;

1.) That I am the sister of the deceased DOMINGA RAZONABLE

TECSON, who died last March 10, 2019 in Lower Limonzo,
Padada, Davao del Sur, Philippines;

2.) That recently it was discovered that her birth year as reflected in
her PSA records is erroneously indicated as 1937, instead of
1938 which is her true and correct birth year as reflected in
her Social Security System records.

3.) That the records of the Social Security System are true & correct.

4.) That I am making this affidavit to submit before the concerned

authority to attest to said fact pertaining to her true date of birth,
which is July 18, 1938 and not 1937.
