AFFIDAVIT OF LOSS
I, CHOI SEULKI, of legal age, single, Korean National, with residence
and postal address at 6P Guevara St., Aurora Hill, Baguio City, after having
been duly sworn to in accordance with law do hereby depose and state that:
1 I am an owner of a CELLULAR PHONE, particularly iPHONE5S;
2 On February 15, 2015 my Phone (iPhone5S) was taken by
unidentified person while walking along Session Road going up to
SM;
3 I exerted diligent efforts to locate the said person to recover my
pone but the same could no longer be found, hence, considered
forever lost;
4 The said phone has not been given nor pledged to anyone for any
purpose whatsoever, nor was it taken or confiscated by the
authorities for any violation of any law or traffic law or regulations;
5 I am executing this affidavit to attest to the truth of my foregoing
statements and to state the fact that my phone (iPhone5s) was
taken by unidentified person and for all legal intents and purposes it
may serve.
