AFFIDAVIT of LOSS

I, VENUS G. GARCIA, Filipino, of legal age, married, and a resident of 628

Magnolia Street, QM Subdivision, Baguio City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and state that:

1) As an employee of PLDT Inc., I was issued a Call-of-Business Landline

Plus SIM with number 423-1560;

2) Last 04 March 2018, as I was enjoying the festivities of Panagbenga with

my family along Harrison Road, I failed to notice that the phone containing the
aforementioned SIM card was no longer amongst my possessions;

3) It is possible that I was pickpocketed or the said phone and SIM fell out of
my pocket, resulting in their loss or misplacement;

4) Diligent effort to locate and search in places where the said phone and SIM
might be were exerted but the same were nowhere to be found;

5) I am executing this affidavit to attest to the truth of the foregoing facts and
request for a replacement of the SIM card and possible retention of the number
mentioned above.
