AFFIDAVIT

I, JOSEPH MARTIN GUTIERREZ LEGASPI, Filipino, of legal age, single and a

resident of Ipil Street, Maharlika West, City of Tagaytay City, Cavite after having been duly
sworn to in accordance with law, do hereby depose and say that:

1. I was born on July 18, 1999 at Quezon City, Metro Manila;

2. My mother’s name is GLORIA LUMANIAN GUTIERREZ and my father’s name is

JOSEPH BRUCE RIOS LEGASPI;

3. I found out that in my PSA Certificate of Live Birth, my second name “MARTIN” is
blurred and unreadable;

4. I am attesting to the fact that my true and actual name is JOSEPH MARTIN
GUTIERREZ LEGASPI, as can be substantiated by other documents;

5. I am further executing this affidavit to clarify the details in my PSA Certificate of Live
Birth, to update my records in my company, City of Dreams and for whatever legal
purpose it may serve;
