AFFIDAVIT OF
ATTENDANT OF BIRTH
I, EMILIA TODENIO, of legal age, Filipino citizen, single, and a resident of Brgy.,
Urdaneta City, Pangasinan, after having sworn to in accordance in law, hereby, depose and state
that:

1. I am in in the practice of helping deliver human infants;

2. On 12 July 2015, I attended the birth of Karen G. Solis, who was the child of Pedro D. Solis
and Nida F. Gonzales;

3. I understand that in order that Karen’s certificate of live birth and the delayed registration
be finalized, an affidavit of Attendant of Birth is required.

4. In compliance with the abovementioned requirement, I am executing this affidavit, and

we are executing this affidavit for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 5th day of February
2020 at the City of Urdaneta.
