JOINT AFFIDAVIT OF DOMESTIC PARTNERSHIP

We, NICHOL JOHN S. MABALE and HONEY GRACE B. LLENOS, both Filipinos, of legal age,
and residents of Unit 207, Rufa Residences, Tres Borces St., Mabolo, Cebu City, Philippines,
having been duly sworn in accordance with law, hereby depose and state:

1. That sometime in January 2017, we began a romantic relationship and started to live
and treat each other as husband and wife living under one roof up to the present
without the benefit of marriage;

2. That during said relationship, neither of us engaged in another relationship with any
other individual;

3. That our cohabitation is continuous and uninterrupted from January 2017 up to the
present, which is more than five (5) years with no child;

4. That neither of us suffered from any legal impediment to marry within such
continuous and uninterrupted period;

5. That we have been living as husband and wife for at least five (5) years with no child;

6. That this Affidavit is executed to certify or attest to the truth of the foregoing facts
and for whatever legal purpose that this Affidavit may serve.
