JOINT AFFIDAVIT OF ABANDONMENT

We, LUZVIMINDA D. GUGOL of legal age, married, Filipino and

currently residing at Upper Patiis, Barangay Malanday, San Mateo, Rizal,
Philippines, and NATHALIE L. CAÑETE, of legal age, widowed, Filipino and
currently residing at Fernandos Comp. Patiis Road, Barangay Malanday, San
Mateo, Rizal, Philippines. After having sworn in accordance with the law, do
hereby depose and STATE:

1. That we personally know ELSIE TORRES JIMENO and ROGELIO

CADIZAL JIMENO JR., who were legally married under the laws of
the Republic of the Philippines;

2. That according to our personal knowledge and information the above-

named spouses were blessed with two children namely: ROSIE ANNE
TORRES JIMENO and ROSHIELLE TORRES JIMENO. A copy of their
Certificate of Live Birth is hereto attached as ANNEX “A” and ANNEX
“B” respectively;

3. THAT the above-mentioned children were abandoned by their father

ROGELIO CADIZAL JIMENO JR. since year 2000 and since that time
has not communicated with them nor extended any financial support.
A copy of the Affidavit of abandonment and Solo parent certification is
hereto attached as ANNEX “C” and ANNEX “D” respectively;

4. That since the abandonment, their mother has been solely supporting
them;

5. That their mother ELSIE TORRES JIMENO passed away on June 27

2022. A copy of the Certificate of Death is hereto attached as ANNEX
“E”

6. THAT we are executing this affidavit to attest to the truth of the

foregoing facts, particularly, the abandonment of their father, and to
use the same for whatever legal purpose it may serve.

IN WITNESS WHEREOF, WE hereunto affix our signature this 20 th of
