AFFIDAVIT OF ABANDONMENT

I, LAILANI MADRIO PANGAN, of legal age, Filipino citizen and currently residing
at Lot 1 Block 7 Banyuhay Ville, Barangay Silangan, San Mateo, Rizal, Philippines. After
having sworn in accordance with the law, do hereby depose and STATE:

1. THAT out of the relationship with my former common-law partner Ferdinand Sugmit,
were born Laidie Lee M. Pangan, 22 years old;

2. THAT my former partner Ferdinand Sugmit abandoned me and my child in year 2000
and since that time has not communicated with us nor extended any financial
support;

3. THAT Since the abandonment, I have been solely supporting our child;

4. THAT after I was abandoned by my former live-in partner, Ferdinand Sugmit, I met
and had developed a dating relationship with Ryan Albania;

5. THAT out of my relationship with Ryan Albania, were born Ryan P. Albania Jr., 12
years old;

6. THAT in 2010, Ryan Albania has abandoned me and our child. After abandonment,
Ryan Albania has not communicated with us nor extended any financial support,
hence, I had been supporting our child solely;

7. THAT after I was abandoned by my former live-in partner, Ryan Albania, I met and
had developed a dating relationship with Jonathan Rañola;

8. THAT out of my relationship with Jonathan Rañola, were born Prince Ace Pangan, 5
years old;

9. THAT in 2021, Jonathan Rañola has abandoned me and our child. After
abandonment, Jonathan Rañola has not communicated with us nor extended any
financial support, hence, I had been supporting our child solely;

10. THAT I am executing this affidavit to attest to the truth of the foregoing facts,
particularly, the abandonment of my spouse/common-law partners.
