AFFIDAVIT OF WHEREABOUTS

I, RODRIGO LEYBAG GELLIDO, Filipino, of legal age and a resident of 23Rd 3

Lingahan St. Malanday, Valenzuela City, after first having been duly sworn to in accordance
with law, depose and say:

1. That I am the father and legal guardian of PAIDIO ANDREI ABANGAN

GELLIDO, who was born on November 19, 2015 at Valenzuela City;

2. That there is no other judicially appointed guardian over the person or property of
the minor since her biological mother is whereabouts and I am the legal father of the
said child;

3. That I do not have communication and any knowledge about the whereabouts of
her mother Angeli Sebastian Abangan since we got separated sometime on year 2016
up to present time;

4. That since we live separatedly she has disappeared from our lives since then;

5. That my said child doesn’t even recognized her mother, due to we have no personal
knowledge whatsoever of the whereabouts of Angeli Sebastian Abangan;

6. That since Angeli Sebastian Abangan left our family, until as of date, we never saw
her again and no information whatsoever of her whereabouts;

7. Diligent efforts were made to locate the said person but no avail. We have no
contact whatsoever from the relatives and friends of Angeli Sebastian Abangan;

4. That I am executing this affidavit to attest to the truth of all the above facts and
statements and for all legal intents and purposes this may serve.
