AFFIDAVIT OF GUARDIANSHIP

I, SHEENALYN ALBURO DE LOS SANTOS, widow, of legal age, Filipino, and a resident
of Brgy. 15, San Jose, San Nicolas, Ilocos Norte, Philippines, after having been duly sworn to
in accordance with law, hereby depose and states that:

1. I am the mother of MISHA LOREI ALBURO DE LOS SANTOS, GABE ZION ALBURO
DE LOS SANTOS, and CAESHA LOUISE ALBURO DE LOS SANTOS, who were born
on December 21, 2012, December 14, 2014, and August 12, 2018, respectively;

2. My husband MARLO MARIANO DE LOS SANTOS died on February 28, 2022 in

Batac City, Ilocos Norte;

3. My husband was insured and has an insurance policy under SGTI;

4. My said children who have an interest in the said insurance policy are minors and
under my care and custody;

5. I am competent to receive on behalf of my minor son and daughters any amount

due to them by reason of the said policy;

6. I am executing this Affidavit to attest to the truth of the foregoing statements;

and

7. Finally, I am executing this Affidavit for whatever intents and purposes it may
lawfully serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 6 th day of December 2023 at
