1. I am carrying the name WILSON DEHITO NOYNAY since birth. Attached as

evidence herewith is my Negative Certification of Birth and Certificate of
Marriage;

2. However, there is a discrepancy in my name being listed in my son’s, John

Welson Cantillas Noynay, certificate of live birth as WELSON DEHITO
NOYNAY.

3. Both names WILSON DEHITO NOYNAY and WELSON DEHITO NOYNAY

pertains to one and the same person;

4. I am executing this affidavit to attest to the truth, and for whatever legal
purposes it may serve.
