AFFIDAVIT OF DISCREPANCY

I, RHEA MAE DOTILLOS MONTEJO, single, Filipino citizen, with

residence at Barangay Poblacion, Municipality of Tabuelan, Cebu,
Philippines, on oath, states THAT:

1. I am the affiant of herein Affidavit of Discrepancy;

2. When I secured a copy of my Certificate of Baptism from the SAN

ROQUE PARISH, ARCHDIOCESE OF CEBU, I discovered that
the name as indicated therein is “RHEA MAE D. MONTIJO”
instead of “RHEA MAE D. MONTEJO".

3. Since birth, I have been using the name RHEA MAE D.

MONTEJO, particularly in my scholastic records.

4. The names “RHEA MAE D. MONTIJO” and “RHEA MAE D.

MONTEJO" refers to one (1) and the same person; and

5. I am executing this affidavit to attest to the truth of the foregoing

facts and to correct the LAST NAME that appears in my Certificate
of Baptism from “MONTIJO” to “MONTEJO”.

IN WITNESS, I have hereunto set my hands this 24th day of

September 2022, in Municipality of Tabuelan, Cebu, Philippines.
