AFFIDAVIT OF GUARDIANSHIP

I, NORAIMA MASNAR RANGAIG of legal age Filipino, and with residence address and
postal address at East Poblacion, Balo-I, Lanao del Norte, after having sworn to in accordance
with law do hereby deposes and state:

1. That I am the biological mother and guardian of minors, namely: MOHAMMAD HASIB
RANGAIG LIWALUG (born on July 30, 2010), MOHAMMADNOR RANGAIG LIWALUG
(born on July 10, 2009, and MOHAMMAD WALLY RANGAIG LIWALUG (born on July 25,
2011);

2. That I would like to declare and affirm that the abovenamed minors have been under
my care and custody since birth, and all are dependent on me for their daily sustenance
and support;

3. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purposes and intent this may serve.
