AFFIDAVIT OF GUARDIANSHIP

I, MA. ALONA P. BEBERO, of legal age, married, Filipino, with residence

and postal address at Block 1, Lot 8, San Isidro Heights, Brgy. Banlic, City of
Cabuyao, Laguna, Philippines after having been duly sworn to in accordance
with law hereby depose and state;

That I am the guardian of RACHELLE P. VILLAMOR who was born on

August 6, 1999 at Campostela Valley, Davao Orriental from Ricardo Villamor
and Evelyn Pascua;

That as such, I am the guardian of said RACHELLE P. VILLAMOR while

she is living with me and under my custody and I will be the one to sign as
guardian to enter into legal transactions in her behalf;

That I am executing this Affidavit as one of the requirements for the

entrance of said child at Pulo National High School and for whatever legal
purpose it may serve.

accordance with law hereby depose and state;

That I am the aunt of MA. CARLLYN ALFORQUE who was born on

March 1, 1995 at Sta. Cruz, Metro Manila;

That her mother is JOCELYN S. ALFORQUE and her whereabouts is

unknown and cannot be located and he has no father and MA. CARLLYN
ALFORQUE is living with me, under my custody and I am the one who take care

That as such, to support said child and that believing that it would be for
her own interest and benefit, I, hereby signify my written intent that MA.
CARLLYN ALFORQUE is under my custody and I act as guardian of the said
child;

That I am executing this Affidavit to attest the truth of the foregoing fact
and for whatever legal purpose it may serve.

Philippines after having been duly sworn to in accordance with law hereby
depose and state;

1. That I am the biological mother of DANN JIO HARLEY V. CASSITY

who was born on July 25, 2011 at Jose Pabella Memorial Hospital,
Metro Manila;

2. That my son name above mentioned will travel in United States of

America (USA);

3. That I am giving full consent to my son to travel in USA to be

accompanied by his father, BRAD ALLEN CASSITY;

4. That for this purpose, we have sufficient and adequate financial

capability to defray all expenses during and for the duration of his
travel/stay in USA at which time he will not become a public charge
that would embarrass our family who is currently staying in the USA;

5. That I execute this affidavit as one of the requirements to travel

abroad andfor petition purposes of my son name above-mentioned and
to affirm the truth of the foregoing facts for whatever legal purpose it
may serve.
