AFFIDAVIT OF CONSENT

I, NOEL LAZO GUZMAN, of legal age, Filipino, married and a resident

of Blk 7, Lot 8, Lincoln Lane, Villa Celina, Las Piñas City, after having been
duly sworn in accordance with law, do hereby depose and state that:

1. I am the Father of BEATRICE NICOLE GUZMAN, born on

___________________ in ___________________ City and who is
currently residing with me at Blk 7, Lot 8, Lincoln Lane, Villa Celina,
Las Piñas City;

2. Her mother, PABLITA TRINIDAD, is currently residing at

__________________________. My daughter BEATRICE NICOLE
GUZMAN intends to celebrate the Holiday Seasons with her mom
in Bahrain from December 20, 2011 to January 7, 2012;

3. In connection with this, I hereby confirm my consent to the travel

of my daughter to Bahrain;

4. As the parent of my minor child, I shall provide her with all the
financial support that she shall be needing while traveling abroad,
such as, but not limited to, payment of roundtrip airfares, hotel or
lodging accommodations, allowances for food and other related
expenses;

5. This Affidavit is being executed for purposes of giving consent to

the travel of my daughter to Bahrain and for whatever legal
purpose this may serve.
