AFFIDAVIT OF SOLO PARENT

I, ALMADETTE L. VILLANUEVA, of legal age, Filipino, solo parent, and residing at

2224 A. Cabo Street Gagalangin, Tondo, Manila after having been duly sworn to in accordance
with law, hereby depose and state:

1. That since October 2010, JEFFREY B. COMETA and I had been living together as
husband and wife without the benefit of marriage;

2. That during our lived-in marital relationship, the following children were born out
of wedlock:

3. That JEFFREY B. COMETA and I, had been separated on July 2016;

4. That at the present time, I have no husband and as a solo parent, I am taking care
and I am presently having the custody of our two (2) child mentioned in No. 2 of
this Affidavit;

5. That this Affidavit is being executed for purposes of complying with the
requirements of the Department of Social Welfare and Development (DSWD) for the
issuance of a Solo Parent Identification Number;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and
circumstances and for whatever all legal intents and purposes this instrument may
serve.
