AFFIDAVIT

(Support and Consent/Parental Travel Permit)

I, ____NORA R. VENTURA_________ of legal age, MARRIED, FILIPINO and a resident of

GUMAMELA ST PALTOK ANGAT BULACAN and with tel. no. _______________, after
having been duly sworn to in accordance with law do hereby depose and state:

1. That I am the biological mother and/or person exercising parental authority

over minor BETZAIDA R. VENTURA

2. That the child will travel to SOUTH KOREA for the following reason/s:
TOURISM/LEISURE;

3. That the mentioned child will be leaving on MAY 28 and will stay in the
said country for a period of 5 DAYS and will be taken cared of by my niece,
MONICA MAY RAMOS;

4. That I cannot personally travel with my child due to the following reasons:
Family Business and Home responsibility;

5. That I am giving my full consent for the minor child to travel abroad to be
accompanied by my niece, MONICA MAY RAMOS;

6. That I personally guarantee the financial support of the child in the course
of her their travel and stay outside the Philippines and further guarantee that she
will not in any manner become public ward of any authority of any country she may
happen to pass en route to her destination;

7. That this affidavit was executed for the purpose of attesting to the truth of
the foregoing facts and for whatever legal purpose it may serve; and

8. Affiant further sayeth naught.
