AFFIDAVIT OF DISCREPANCY
AND CORRECTION

I, MARIA CATHERINE OGSONAR OVIEDO, of legal age, Filipino, single, and

residence and postal address at 2108-B2 Molave Street , Jose Abad Santos Avenue, Tondo,
Manila, after having been duly sworn in accordance with law, do hereby depose and state:

1. That I am the daughter of SONNY A. OVIEDO and DYNA I. OGSONAR who was
born on May 19, 1990 in Manila;

2. That in my Certificate of Live Birth my parents date of marriage was recorded as

November 17, 1987 in Manila, which is erroneous:

3. That when I requested for Certificate of No Marriage in the office of Philippine

statistics Authority it was stated that SONNY A. OVIEDO and DYNA I.
OGSONAR has no record in the indices of Marriage;

4. That I am executing this affidavit to attest to the truth of the foregoing facts and
circumstances and for whatever legal intents and purposes this instrument may
serve in the future.
