AFFIDAVIT OF DISCREPANCY
RE: ONE AND THE SAME PERSON

I, JENNIFER C. VEGA, of legal age, Filipino, single and with address at

Paniqui, Tarlac, having been duly sworn in accordance with law, hereby
depose and state:

1. That my name is JENNIFER CAWAGDAN VEGA;

2. That I am the biological daughter of JOSE OBCENA VEGA and

ERLINDA NOOL CAWAGDAN;

3. That I have been using the name JENNIFER CAWAGDAN VEGA,

in all my documents, valid identification card and transactions;

4. That when I obtained a copy of my Certificate of Live Birth issued

by the Philippine Statistics Authority, I discovered that my middle
name appearing thereon was erroneously written as
“CAOAGDAN”. Copy of said Birth Certificate is hereby attached as
ANNEX “A”;

5. That my true and correct middle name is CAWAGDAN;

6. That I am executing this Affidavit to attest and declare the truth of

the foregoing facts and for whatever legal purposes that this
Affidavit may serve.

8. That I have been using the name RODOLFO YAGO PAGARIGAN,

in all my documents, valid identification cards and transactions;

9. That the name “RODOLFO YAGUE PAGARIGAN” as appearing

in my records with Philippine Crop Insurance Corporation and the
name “RODOLFO YAGO PAGARIGAN” as appearing in all my
documents, valid identification cards refers to one and the same
person;

10. That my true and correct name is RODOLFO YAGO

PAGARIGAN;

11. That I am executing this Affidavit to attest and declare the

truth of the foregoing facts and for whatever legal purposes that
this Affidavit may serve.

1. That I am the daughter of the late Juan Villanueva Ancheta Sr.;

2. That my father have been using the name Juan Villanueva

Ancheta Sr., in all his documents, valid identification cards and
transactions;

3. That the name “JUAN VILLANUEVA ANCHETA” as appearing in my

Certificate of Live Birth and the name “JUAN VILLANUEVA
ANCHETA SR.” as appearing in all the documents and
identification cards of my father, refers to one and the same
person;

4. That I am executing this Affidavit to attest and declare the truth of

the foregoing facts and for whatever legal purposes that this
Affidavit may serve.
