AFFIDAVIT OF PARENTAL CONSENT TO MARRIAGE

We, Vanessa S. Apawon and Elly N. Apawon , of legal age, married, Filipino citizens, and
presently residing in Barangay North Dadiangas Manga St. General Santos City, after
having been duly sworn to in accordance with law, do hereby depose and state:

1. That we are the parents of Elissa S. Apawon born in Barangay North Dadiangas Manga
St. General Santos City, on May 2, 1998;
2. That our child is intending to contract marriage with Joseph M. Magallanes in Our
Lady Parish of Good Voyage, General santos City;
3. That we are giving our consent to our child, Elissa S. Apawon, marrying; Joseph M.
Magallanes;
4. That we are executing this Affidavit for the purpose of informing the authorities
concerned of the veracity of the foregoing facts and for whatever legal purposes it may
serve.
