AFFIDAVIT OF NON-FILING OF INCOME TAX

I, MARISTELA A. PANTALEON II, legal age, Filipino, with postal address at #101 Cattleya
Street, Pingkian Village, Brgy. Pasong Tamo, Quezon City, after having been sworn to in accordance
with law, hereby depose and say:

1. That I hereby attest to the fact that I am unemployed and have no other business from which I can
derive any income since January 2019 up to the present;

2. That I did not file my income tax in accordance with the National Internal Revenue Code of the
Philippines, as I am not required by the law to file an income tax return having no income from
the date and year mentioned above;

3. That I am executing this affidavit to attest to the veracity of the foregoing facts, in support to my
scholarship application and for whatever legal purpose this may serve.
