AFFIDAVIT
I, MARY JOY NACION, Filipino, of legal age, married with mailing address at 5 Macaya
Street, Pasong Tamo, Quezon City, after having been duly sworn in accordance with law, hereby depose
and say that:

1. I am the mother and manager of Milkcah Wynne Nacion, Filipino, minor, and a professional child
artist / actress with TIN #______________, duly registered with the Bureau of Internal Revenue under
RDO ___;

2. As a professional child artist, Milkcah Wynne Nacion renders professional services to a number of
clients and that her fees range from ten thousand pesos (P10,000.00) to fifteen thousand pesos
(P15,000.00),

3. Her work conditions are well within the limitations mandated by Philippine Labor laws, issuances of
the Department of Labor and Employment, the Department of Social Welfare and Development, and
other laws and orders of pertinent government agencies;

4. I intend to bill clients of my child by issuing them Official Receipts that are duly registered with the
Bureau of Internal Revenue;

5. The factors I consider in determining the equitable rate of fees charged or to be charged are: (a) the
complexity of the work performed or to be performed; (b) the extent of responsibility assumed; (c)
the volume of work or number of hours spent or to be spent in doing the work; and (d) length or
period of engagement;

6. This affidavit is hereby executed to attest to the truth of the foregoing facts, and in compliance with
BIR Revenue Regulation No. 4-2014 and other pertinent rules, regulations and issuances.
