AFFIDAVIT OF LOSS
I, ____________________, Filipino, of legal age and with address at Kabulusan.
Gen. E. Aguinaldo Cavite, after being duly sworn in accordance with law,
depose and state, that;
1. I am in possession of a Metrobank check issued by Interlink Manpower
Dev’t & Gen. Services, Inc. with check no. 3473327808

2. That said check has been lost for the reason that I have accidentally
dropped said check without my knowledge;

3. Despite further efforts to recover the said check I could no longer recover
the same;

4. I, OLIVER B. NICOLAS, further understand that by presentation of this

affidavit and the issuance of a replacement check that I can be held legally
liable both under criminal and civil laws if I should attempt to cash or
present the above described check to any bank, financial institution,
currency exchange, or any other third party

5. I am executing this affidavit to attest to the truth of the foregoing facts and
not for any unlawful reasons but for the replacement of said check.
