AFFIDAVIT OF EXPLANATION

I, LEON LAQUITE DELA ROSA, Filipino, of legal age, single and residing at
Purok 80 Kaybagal South Tagaytay City after being duly sworn to in accordance with
law do hereby state and declare that:

1. I am currently assigned at Tagaytay Component City Police Station. On

March 1, 2021, a Memorandum was issued in relation to an Order to Explain
from Unit Investigator pertains to my alleged loan;

2. On 23 June 2020, I secure a loan in PSSLAI in order to provide financial

support to my mother and does not have a job at that time. I also supported
my siblings. I also pay some personal debts because the creditors have been
demanding payments. At that time, due to pandemic, I have no other source
but my own salary and the same is not sufficient;

3. I also secured loans in AFPSLAI on 19 July 2020 and AWMSLAI on

September 2020 for purposes of renovating our house and in order to finance
our sari-sari store so that my family can cope up with the consequences of
the pandemic ;

4. I thus, with due sincerity and humility, I, P/T LEON LAQUITE DELA
ROSA, did not intentionally defy the lawful order coming from the higher
headquarters and do hereby ask for your consideration and undertaking
regarding this matter;

5. I execute this Affidavit of Explanation to attest the truthfulness of the

foregoing statement to and for whatever legal purpose this may serve.

AFFIDAVIT OF LOSS

I, MARY JANE H. RAMOS, Filipino, of legal age, single and residing at Salaban, Amadeo,
Cavite, after being duly sworn to in accordance with law do hereby state and declare that:

1. I am the owner of a Metrobank ATM Card (Payroll) with Account No. 1773177800326; My
live-in partner, Mr. KING CARLO S. DERECHO is in possession of the said ATM Card
and placed it in his wallet;

2. On January 29, 2021, while I was with my live-in partner and about to withdraw from a
Metrobank ATM Machine, we suddenly noticed that the wallet of my live-in partner
containing the abovementioned ATM card can no longer be found from his back pocket;

3. I exerted all my diligent efforts to recover the same but to no avail;

4. I executed this affidavit to attest to the truth and veracity of the foregoing statements, to
apprise proper authorities for the loss of the above ATM card and to request for its replacement and
for all legal intents and purposes this may serve.

I, DOMINEE GEORGIA J. DECHAVEZ, Filipino, of legal age, single and residing at

Kaybagal Central, Tagaytay City, Cavite, after being duly sworn to in accordance with law do
hereby state and declare that:

1. I am an owner and holder of a Globe Prepaid Sim Card with Cellphone Number 0945-
778-8544;

2. I placed the said sim card at the back of my cellphone (supported by the cellphone
casing) but was not inserted in the cellphone sim slot;

3. During the second week of January 2021, I suddenly noticed that the said sim card is
already gone and nowhere to be found from the back of my cellphone;

4. I exerted all my diligent efforts to recover the same but to no avail;

4. I executed this affidavit to attest to the truth and veracity of the foregoing statements, to
apprise the proper authorities of the loss of the above Globe Sim Card and to request that my
GCASH ACCOUNT under Globe Telecommunications be transferred to my current Smart
Telecommunications Cellphone Number 0949-968-2124.
