I, ARFIE RICO D. GONZALES, Filipino citizen, of legal age, married

and with present residence and postal address at 11 Payapa Road, Galicia 3,
Mendez, Cavite after being duly sworn to in accordance with law, hereby
depose and state:

That I am the lessor/ landlord of Mr. Christian Toledo;

That he has been renting one of the 3 apartment units located at 11 Payapa
Road, Galicia 3, Mendez, Cavite from June 2019 until the present;

That his monthly rental fee is Php 6,000.00;

That due to the pandemic crisis, he lost his employment and was not able to
pay the rental fees for four consecutive months (April, May, June, and July);

That I have likewise sent him a formal demand to vacate the property
attached herewith;

I am executing this affidavit in good faith and without any intention to

defraud or deceive; and

As such, I am executing this Affidavit to attest to the truth of the foregoing

and for other reliefs as may be just and equitable.
