I, JOSEPH R. ANCHETA, of legal age, married, Filipino, with

residence and postal address at B1 L5 P5 Narra Avenue, Narra Homes, Pag-
Asa I, Imus City, Cavite, after having been duly sworn in accordance with law,
deposes and states, THAT:
1. I am married to ESTRELLA C. ANCHETA of the same address as
stated above;

2. I have been an Overseas Filipino Worker (OFW) but was constrained

to stop due to non-renewal of contract;

3. Ever since my return to the Philippines, I never found employment

and remained unemployed up to the present time;

4. This affidavit is being executed for the purpose of attesting,

confirming and affirming the veracity of the foregoing facts and for
whatever legal purpose this affidavit may serve best.

Asa I, Imus City, Cavite, after having been duly sworn in accordance with law,
deposes and states, THAT:
1. I am married to JOSPEH R. ANCHETA of the same address as stated
above;

2. I am a retired employee since February 4, 2017 and is presently

receiving a monthly pension of P12,230.00 from the Social Security
System (SSS).

3. Such pension is being utilized solely for my maintenance medicine

and chemotherapy since I was diagnosed with breast cancer;

4. This affidavit is being executed for the purpose of attesting,

confirming and affirming the veracity of the foregoing facts and for
whatever legal purpose this affidavit may serve best.
