AFFIDAVIT

I, NILO D. ARONG, of legal age, Filipino, married and a resident of Yati, Lilo-
an, Cebu, Philippines, after having been sworn in accordance with law, do hereby
depose and state:

1. THAT I am the son of Fidel Arong and Lidelina Dagatan;

2. THAT I was born on January 24, 1970 at Santiago, San Franciso, Cebu,
Philippines;

3. THAT in my Birth Certificate issued by the Philippine Statistics Authority, the

entry for both Male and Female has been marked and indicated to be my SEX;

4. THAT the said marking for both Male and Female is incorrect and I would like to
have this rectified;

5. THAT my correct SEX is MALE;

6. THAT I have been entering my SEX as MALE in all my records, documents and
transactions, knowing this to be true and correct; and

7. THAT I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
