AFFIDAVIT

I, JOSEPHINE LAPIÑA ABINES, Filipino, married of legal age, and a

resident of Labugon, Mandaue City, after having been duly sworn to in
accordance with law hereby depose and state, THAT:

1. I am currently working at DEPED as TEACHER III located in Mandaue

City;
2. When I secured a copy of my Certificate of Baptism from the
Archdiocesan Shrine of St. Catherine of Alexandria’s Office (attached as
Annex A) and a copy of my LCR Birth Certificate (attached as Annex B), I
found out that my baptismal as indicated is October 21, 1978;
3. That my real name, as shown in my NSO Birth Certificate is Josephine
Alcover Lapiña (attached as Annex C);
4. I am executing this affidavit to attest to the veracity of the foregoing
statements and for whatever other legal purpose it may serve.
