I, RHAYZIANE MAE B. RIVERA , of legal, Filipino and resident of 1 Caragay

St., Bgy. Damayan , SFDM, Quezon City, after first being sworn to in accordance with
law, depose and say:

1. That my given name in my Certificate of Live Birth is RHAYZIANE MAE

BALON RIVERA and have used the same in all my records, dealing and
activities ;

2. That however, my name appears from the master payroll of the

CityTreasurer’s Office of Quezon City as RHAZIANE MAE ( letter Y was
omitted) while it should be spelled as RHAYZIANE ;

3. That the discrepancy was purely human error and excusable negligence
committed by the clerk who encoded the same ;

4. That I am executing this affidavit to attest to the veracity of the foregoing

discrepancy in my given name above-stated in order t request the authorities
concerned to rectify the same ;

5. That my given name in my Certificate of Live Birth is ASHKIA MICKAEL

GONZALES EBRO and have used the same in all my records, dealing and
activities ;

6. That however, my name appears from the master payroll of the

CityTreasurer’s Office of Quezon City as ASHKIA MICHAEL ( letter K was
omitted) while it should be spelled as MICKAEL ;

7. That the discrepancy was purely human error and excusable negligence
committed by the clerk who encoded the same ;

discrepancy in my given name above-stated in order t request the authorities
