AFFIDAVIT OF EXPLANATION

I, Warlito A. Mendoza II, Filipino citizen, married, of legal age and a resident of B3, L7,
Sherwood Heights Subd. Brgy. Pasong Tamo, Quezon City assigned at Station Health Unit ,
Police Station 3, Talipapa, Quezon City.after having been deposed and state that:

1. That on March 13, 2021 , I attended a meeting with the DASCOTF upon
instruction by the Deputy District Director For Administration. In the said
meeting, all of the attendees where directed to sign their name in the
attendance sheet and indicate their contact number on said attendance sheet.

2. That because of my failure to indicate my contact number in the said attendance

sheet, I am now being charged with violation of the 2016-002 Rule 21 Sec. 1,
Para B (1) (d) which states :

(d) Fail to comply with any lawful Order or instruction of Superior Officer
or Chief of Police.

3. That I have no intention of disobeying a lawful Order/Instruction of the Superior

Officer. Because of inadvertence that I failed to indicate my contact number. At
that time I was signing the attendance sheet, the attendees where all lined up
and practicing social distancing. The meeting was about to start that I was in a
hurry to sign, thinking I can go back to it again later before the meeting will end.

4. However, at the end of the meeting, I went straight to my duty at Station 3,

without signing my contact number.

5. That I am executing this Affidavit in compliance with the Order to Explain and I
am appealing to the proper authorities to give me another chance. I am willing
to make for it and to prove myself worthy.
