I, NEIL ALLAN CANTAL DE LEON of legal age, Filipino, married, with

residence and postal address at 25 Viceroy Street BF Homes Phase 4, Talon
Tres, Las Pinas City, after having been duly sworn to in accordance with law
hereby depose and state that;

I was employed by LOZADA COMMERCIAL CORPORATION with office

and business address at Km. 20 Alabang-Zapote Road, Talon Uno, Las Pinas
City since January 2000 up to April 2012.

The said employer recently paid for Pag-IBIG Fund contributions from
January 2000 up to October 2006 as part of the amnesty they availed but
upon verification however, I gathered the information that my employer failed
to remit to the Fund the contribution due for the periods November 2006 up
to; April 2012 as part of the NLRC Decision which was won in my favor.

This affidavit is executed to the truth of the foregoing facts, and to

manifest my willingness to act as witness in any legal proceedings that may be
instituted against my employer for violation of the Pag-IBIG Fund Law,
Republic Act 9679 and its Implementing Rules and Regulations.
