AFFIDAVIT OF NON-OPERATION/CLOSURE

I, MARK AARON S. VESTIL, of legal age, single, Filipino and with

residence at Aberdeen St., Suburbia North, Maimpis, City of San
Fernando, Pampanga, after having been duly sworn in accordance with
law, depose and state that –

1. I am the proprietor of CAM-CAS PUB AND RESTAURANT, a

sole proprietorship duly established under and by virtue of Philippine
laws, with office address at 2/f Lot 40, Blk. 48, Fil-Am Friendship
Highway, Pampang, Angeles City;

2. CAM-CAS PUB AND RESTAURANT suffered substantial

losses and did not earn income in 2019. Hence, I decided to stop its
operation and close the business effective 2020;

3. I have executed this affidavit to prove and establish the

foregoing facts, to attest to their truth and for all other legal intents and
purposes;
