AFFIDAVIT OF SUPPORT AND CONSENT

I, ALTHEA D. ALONZO, mother of Jethro Alonzo , of legal age , and resident No. 72 Sct.
Alcaraz corner Pao Sts ., Bgy. St. Peter , after having been duly sworn to in accordance with law,
do hereby depose and state:

1. That I am giving full consent and authority to apply for passport with the Department of
Foreign Affairs and to apply for appropriate visa with the U.S. Embassy in Manila on
behalf of my child JETHRO ALONZO;

2. That I am/we are giving full consent for my child to travel from the Philippines to
Australia with traveling companion, , Rosario D. Alonzo who is my mother;

3. That I am/we are giving consent to the guardian to procure tickets for the travel of my
child;

4. That I am/we are willing and able to maintain and support my child financially and
guarantee that he/she/they will not become a public charge or burden during his stay in
the Australia;
