AFFIDAVIT

I, JONATHAN C. DIHAYCO, of legal age, Filipino Citizen, and a resident of Lapu-

Lapu City, Cebu, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and say that:

1. I personally know FELISA SALUDAGA DIHAYCO who died last April 23, 2020
because the latter is my grandmother;

2. Aside from my other siblings and cousins, I am the nearest surviving kin of the
deceased person and that I was given authority by the rest of my family to secure a
copy of my grandmother’s death certificate;

3. I am in the process of collating documents in relation to my grandmother’s death for

personal purposes;

4. I am executing this affidavit to attest to the truth of the foregoing statement and in
support of my intention to secure a copy of the Certificate of Death of my deceased
grandmother;

5. I further execute this affidavit to attest to the truthfulness and veracity of my

statement above;
