    The undersigned is a current 3rd year student of the University of San


    Carlos taking up Antropology. That recently, it was discovered that my name
    under the school records appear to be DAMIE-ANN BERAME ABALLE instead of
    DAMIE-ANN CALUPAS BERAME, my birth name.

    Having said that, I would like to request for a correction of my name in the
    school records from DAMIE-ANN ABALLE to DAMIE-ANN CALUPAS BERAME.
