AFFIDAVIT OF CONSENT AND SUPPORT TO TRAVEL

I, DJ Pendilla, of legal age, Filipino, married to Katherina S. Bernardo, and a resident

of Room 143, ABS CBN Condotel Tower, Mother Ignacia, Quezon City, after being duly
sworn to in accordance with law do hereby depose and say, THAT:

1. I am the lawful parent of Juan Pendilla, who was born on October 22, 2009, and
whose birth certificate is registered under number 2009-1234 issued by the
Philippine Statistics Authority;

2. I hereby declare that I am financially capable and willing to provide support to Juan
Pendilla during his travel to HONG KONG. This support includes but is not limited
to accommodation, transportation, and other necessary expenses during the entire
duration of the trip;

3. I consent to Juan Pendilla traveling to HONG KONG with his grandparent

ROSARIO PENDILLA from May 2-4, 2024. I understand that during this time, Juan
Pendilla will be under the care and supervision of Rosario Pendilla, and I grant full
authority to make decisions on her behalf regarding his welfare and well-being;

4. I acknowledge that Rosarion Pendilla will accompany Juan Pendilla during his
travel and will ensure his safety and security throughout the journey;

5. I undertake to bear all financial responsibilities and liabilities that may arise during
Juan Pendilla’s travel to Hong Kong and ensure that he will not become a public
charge or burden to any entity or individual during the said period; and

6. As such, I affirm that all the information provided herein is true and correct to the
best of my knowledge and belief.
