AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, JUAN DELA CRUZ and PEDRO GARCIA, both of legal age, Filipinos,
married and residents of Zone 7, Kasipagan Street, Brgy. Del Monte, Quezon City
and No. Zone 8, Maligaya Street, Brgy. Del Monte Quezon City, after having been
sworn to according to law hereby depose and say;

1. That we personally know ANGELO FLORES who is the son of spouses Tino
Flores and Angela Flores;

2. That in the Certificate of Live Birth of ANGELO FLORES, his date of birth is
April 1, 2000, whereas in his valid identification cards, SSS Records, Driver’s
License, Voters Identification Records and other pertinent documents, his
date of birth is April 2, 2000;

3. That the reason why in those documents, he indicated his birth date to be
April 2, 2000 and not April 1, 2000, which is his correct birth date was
because he grew up in a foster family and that it was his foster parents who
informed him that April 2, 2000 was his birthday;

4. That it was only later when he secured a copy of his Birth Certificate that he
found out that April 1, 2000 was his real date of birth;

5. That we know the foregoing facts being long time neighbors and friends of
ANGELO FLORES and his family;

6. That we are not related to him either by consanguinity or affinity and has no
interest in whatever this affidavit could be of use either directly or indirectly;

7. That we execute this affidavit to attest to the truthfulness of the foregoing

facts and to establish that the date of birth of ANGELO FLORES is April 1,
2020, and for whatever legal purpose it may serve.

IN WITNESS WEREOF, we have hereunto affixed our signature this 6th day
