JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, FABULOUS CANDELARIO ALVARO CAPUTOL and FELISA COLES

LIBOTLIBOT, both of legal age, Filipinos, and both residents of Danao City, after having been
sworn in accordance with the law hereby depose and state;

1. That we know TRICIA MAE PEÑAS TOÑARES who was born on November 11,
2007 at Danao City, Cebu to ALLAN DUYAG TOÑARES and MARICEL MANUEL
PEÑAS;

2. That due to inadvertence and failure to have immediate follow-up on the part of his
parents and the prevailing conditions then, her birth was not reported to and
registered with the Office of the Local Civil Registrar of Danao City immediately
thereafter;

3. That we have firsthand knowledge of the foregoing facts because we have been
close friends and neighbors with her parents for a long time;

4. That we are not related to TRICIA MAE PEÑAS TOÑARES and her parents and we
do not stand to benefit from the registration of her live birth;

5. That we are executing this Joint Affidavit to attest to the truth of all the foregoing
facts, in connection with the application for the late/delayed registration of birth of the
said TRICIA MAE PEÑAS TOÑARES with the Office of the Local Civil Registrar of
Danao City, and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 1st day of June 2019 at
Danao City, Philippines.
