Zamboanga del Sur, after having been duly sworn to in accordance with law, hereby disposed

Zamboanga del Sur.
2. That I have duly submitted the certified photocopy each of the following documents for
my appointment, to wit:

experience Sheet.
b. Certified photocopy of valid and updated PRC license/ID
c. Certified photocopy of Certificate of Eligibility/Rating
d. Certified photocopy of Transcript of Records (TOR) and Diploma including
post graduate degrees
e. Photocopy of Certificate/s of training
f. Photocopy of Certificate/s of Employment and Service Record
g. Photocopy of latest appointment
h. Certified photocopy of Police Clearance
i. Certified photocopy of NBI Clearance

3. That I am executing this omnibus sworn certification to assure authenticity and veracity
of all documents submitted,
4. That I assume full responsibility and accountability on the validity and authenticity of the
documents submitted and I understand that any violation will automatically disqualify me
from the selection process.
