accordance with law, do hereby depose and say:

1. That we personally know DONYA CI ELO FIERRO of

Sagayaden, Cabugao, Ilocos Sur, bei ng an acquaintance for a
long period of time;
2. That we declare, confirm and affirm that she was born at
Cabugao, Ilocos Sur on August 15, 1969;
3. That she has been using the said birth date in all her
documents;
4. We are not related by affinity or consangui nity to the above-
mentioned person; and
5. That we are executing this affidavit to declare the truth of the
foregoing facts and for whatever legal purpose it may serve.
