AFFIDAVIT OF ACKNOWLEDGMENT OF DEBT

WITH PROMISSORY NOTE

I, GELMER S. GERONIMO, of legal age, Filipino citizen, and a resident of

Brgy. 53, Rioeng, Laoag City, Ilocos Norte, Philippines under oath, hereby acknowledges
the receipt of the amount of PESOS: NINETY-SIX THOUSAND (P96,000.00) from
ALLAN M. SANTIAGO, of legal age, Filipino citizen, and a resident of Raraburan,
Laoag City, Ilocos Norte. Vitally, I hereby undertake to pay said amount to ALLAN M.
SANTIAGO in accordance with the following schedule of payment:

On or before June 25, 2023 Php. 12,000.00
On or before July 02, 2023 Php. 12,000.00
On or before July 09, 2023 Php. 12,000.00
On or before July 16, 2023 Php. 12,000.00
On or before July 23, 2023 Php. 12,000.00
On or before August 6, 2023 Php. 12,000.00
On or before August 13, 2023 Php. 12,000.00
On or before August 20, 2023 Php. 12,000.00

That in case of my default in the payment with the said schedule, I will bear all
the expenses to be incurred by ALLAN M. SANTIAGO in the enforcement of this
obligation.

ACKNOWLEDGEMENT
