AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, ALEJANDRO SANO SAGOSOY, Filipino, of legal age, and a

resident of Purok Mangga II, Soong, Tago, Surigao del Sur, after
having been duly sworn to in accordance with law, depose and state:

1. That I am the biological father of ALIANNAH

CUARTERON SAGOSOY who was born outside of wedlock on
October 26, 2020 at Tago, Surigao del Sur;

2. That at the time of birth of said child, I was not married to

her mother, Rea Bautista Cuarteron;

3. That I acknowledge my paternity/affiliation to the said

child;

4. That I am executing this Affidavit to attest the foregoing

facts and for whatever legal purposes this may serve.
