AFFIDAVIT OF NON-RENTAL
I, EVANGELINE GUERRERO ANDALES, of legal age, Filipino, married and a
resident of Bonifactio St., Brgy. 2, Silay City, Negros Occidental, Philippines, after having
been duly sworn to in accordance with law hereby depose and say:

1. That I am the registered owner of EJGB BAKESHOP, registered with the

Deparment of Trade Industry under Business name No. 1321309 and with business address
at Bonifactio St., Brgy. 2, Silay City, Negros Occidental, Philippines covered by Transfer
Certificate of Title No. 40726 registered to my biological brother and his legal spouse’s
name, CLEMENTE R. GUERRERO and MARY ANN GUERRERO.

2. That I am not paying any rental fee or any lease fee for the operation of my
said bakeshop in the above-mentioned business address.

3. That currently I am processing, securing all necessary license and permits for
the said bakeshop at the Local Government of Silay City and they require me to submit this
affidavit for compliance purposes.

4. That I am executing this affidavit to attest to the truth of the foregoing facts to
the best of my own knowledge and based on authentic records at hand and for the purpose of
declaring that I am not paying any rental fee.
