AFFIDAVIT
I, ARGENE VAMANA SANTOS CAMPOSAGRADO, Filipino, of legal age, married and

residing at Lot 7, Block 8, Padre Gomez St., Gabriela Hills Subd., Brgy. Tubuan 1, Silang,
Cavite, after having been duly sworn to in accordance with law do hereby depose and say that:
1. On April 22, 2022 at about 10:40 o’clock in the morning, I went to Silang Municipal
Police Station, Silang, Cavite to file a complaint against GLADYS FERRIOL LUCES
a.k.a Jayden Ibon. A copy of Police Report is hereto attached and made integral part
hereof;
2. In the said Police Report, I also reported that said person used three (3) G-cash
accounts as follows:

3. I reported the said G-Cash Accounts bearing the aforesaid mobile numbers to G-
Cash Hotline Number 2882 concerning the online investment scam of GLADYS
FERRIOL LUCES a.k.a Jayden Ibon to block the aforesaid accounts;
4. However, due to the foregoing reason, the aforesaid accounts were blocked by G-
Cash and one (1) Leen Perez Mastrile (Leen M) with G-Cash Mobile Number 0942-
296-0992 also reported to G-Cash Hotline Number 2882 because she was not able
to use her G-Cash account for her cash in and out for her business Mas-Perez Sari-
Sari store;
5. The G-Cash notified Leen Perez Mastrile and emailed her a copy of the Police
Report which was reported by me concerning the online investment scam of
GLADYS FERRIOL LUCES using her G-Cash Mobile Number 0942-296-0992 to
cash out the money for my investment to GLADYS FERRIOL LUCES;
6. Thus, Ms. Leen Perez Mastrile went to the Silang Municipal Police Station, Silang,
Cavite to verify the Police Report and she was advised by Police Staff Sergeant
Teddy C. Labampa to go to the Brgy. Tubuan I, Silang, Cavite to make an amicable
settlement to me;
7. Luckily, we saw each other in the Brgy. Tubuan I, Silang, Cavite to report the said
online scam and she told me and explained to the Barangay that her G-Cash
account was used by GLADYS FERRIOL LUCES to cash out the money which I
transferred to her account for my online investment to GLADYS FERRIOL LUCES;
8. Thus, I am requesting to the proper authorities concerned to activate again the G-

Cash Account of Leen Perez Mastrile bearing Mobile Number 0942-296-0992
because she already explained to me and to the Barangay that she was not involved
in the online investment scam made by GLADYS FERRIOL LUCES and I will not file
a case against Leen Perez Mastrile as she was also a victim of GLADYS FERRIOL
LUCES;
9. I executed this affidavit to attest to the truth and veracity of the foregoing statements
and for all legal intents and purposes this may serve.

thru LBC express courier containing our said passport. However, upon reaching home
after my work, I immediately open the parcel from LBC containing our Philippine
passport.
3. Unfortunately, due to excitement in signing my own Passport, I was not able to check
the same, thus, I accidentally signed the passport of my sister Mary Dian Amita
Legados with passport number P2871447B issued on 28 August 2019 by DFA Manila
and valid until 27 August 2029 while my passport number is P2873690B issued on 28
August 2019 by DFA Manila and valid until 27 August 2029;
4. I executed this affidavit to attest to the truth and veracity of the foregoing statements; to
apprise the proper authorities concerned that I mistakenly signed the Philippine
Passport of my sister Mary Dian Amita Legados and to request for the replacement of
the same and for all legal intents and purposes this may serve.
