AFFIDAVIT

I, GEMMA L. OCON, of legal age, married, Filipino, resident of Brgy.

Navarro, Surigao del Norte, after having been sworn to an oath in
accordance with law, hereby depose and say:
1. I am at present the District in Charge of Socorro East District,
Schools Division of Siargao, Socorro, Surigao del Norte;

2. That last September 6,2006, a fire incident happened at Socorro

Central Elementary School thereby gutting down the school
building consisting the Socorro East District Office, ALS Office,
Planning Office, School Canteen including all the School and
District Documents/Records since 1973 to School Year 2005-2006;

3. That as a consequence of the abovementioned incident, Form

137-E of JORDAN C. DIGAL, male, who was officially enrolled in
Socorro Central Elementary School from Grades I to VI and
graduated on school year 2005-2006, could not be found;
4. That I execute this Affidavit to attest to the truthfulness of the
foregoing statements and for all legal intents and purposes it may
serve.
