AFFIDAVIT OF IDENTITY OF PROPERTY

I, SOFRONIO P. RAMOS, JR., of legal age, Filipino, and a resident of Brgy. Asuncion, Maasin
City, Southern Leyte, Philippines, after having been duly sworn to in accordance with law, depose and
say:

1. That I am the vendee of a two hundred seventy-seven square meters (277 sq. m.) parcel(s) of
land registered under the name of the late Anselmo Montalbo, situated at Asuncion, Maasin
City, Southern Leyte, more particularly described as:

a. Lot No. 1857-B-12 covered by TCT No. 117-2022000353 with an area of One Hundred
Sixteen Square Meters ( 116 sq. m.)
b. Lot No. 1857-B-13 covered by TCT No. 117-2022000354 with an area of One Hundred
Fifteen Square Meters ( 115 sq. m.)
c. Lot No. 1857-B-14 covered by TCT No. 117-2022000355 with an area of Forty-Six Square
Meters ( 46 sq. m.)

2. That upon the death of Anselmo Montalbo, he left the said parcel(s) of land to his sole legal
heir, his wife, Carina R. Tagra;

3. That Carina Tagra sold the above-described parcels of land to me, as evidenced by the Deed of
Sale of Real Property, dated May 28, 2012, notarized by Atty. Rafael Cromwell T. Gerong, per
Doc No. 244, Page No. 49, Book No. XIV, Series of 2012, of his book of Notarial Register;

4. That the three (3) parcels of land are still in the name of the late Anselmo Montalbo;

5. That since the time I acquired the property, I exercised all the rights of ownership and
possession, including the introduction of improvements thereon;

6. That in view of the above circumstances, I hereby confirm, attest and affirm that the above
parcels of land are absolutely owned by me, free from all liens and encumbrances;

7. That I am executing this affidavit to attest to the veracity of the foregoing, and for whatever
legal purpose this may serve.
