AFFIDAVIT of UNDERTAKING

I, JOHN GALIREN MADERA ESQUIVEL, Filipino, of legal age, single, with

residence and postal address at number 9002, Mahogany Corner Bermuda Country,
Barangay Mabolo I, City of Bacoor, Cavite, after having been duly sworn to in
accordance with law, hereby depose and state :

1. That my mother, ESMERALDA M. HUMPHREY is the registered owner of the

parcel of land located at Barangay Navarro, City of General Trias, Cavite,
Philippines, which contains a total area of SEVEN HUNDRED FIFTY (750)
square meters and covered by Transfer Certificate of Title with number T-
1164269 of the Registry of Deeds for the Province of Cavite .

2. That I am constructing/building a house on the said parcel of land which is

registered under the name of my mother, ESMERALDA M. HUMPHREY;

3. Thus I hereby assure and guarantee under oath, that I have been duly
authorized by my mother, ESMERALDA M. HUMPHREY to occupy the
above-mentioned parcel of land and to construct/build a house on the said
property/parcel of land;

4. That I am voluntarily and knowingly executing this affidavit to attest to the

truth of the foregoing declaration and for such other legal intents and
purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 12th day of

September, 2019 at City of General Trias, Cavite, Philippines.
