JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, CRIS KARREN ROMEROSA, of legal age, Filipino, and resident of Looc, San
Remigio, Cebu and LEONILA A. QUITA, of legal age, Filipino, and resident of Punta
Princesa, Cebu City, on oath hereby state:

1. That we are the good friends of DEMOCRITO PRATO GACHO and we have known
him for a long time;

2. That we are not related by consanguinity or affinity to said DEMOCRITO PRATO

GACHO;

3. That the real and legal birth date of said DEMOCRITO PRATO GACHO is “JULY
28, 1984”, as annotated on his Certificate of Live Birth;

4. That the DEMOCRITO PRATO GACHO whose birth date reflected in VSMMC
records of “JULY 28, 1985” and the DEMOCRITO PRATO GACHO with birth date
of “JULY 28, 1984” reflected in his Certificate of Live Birth, refer to one and same
person who is our friend, DEMOCRITO PRATO GACHO whose correct birth date
is JULY 28, 1984;

5. That we execute the foregoing to attest to the truthfulness and veracity thereof and for the
purpose of complying the PhilHealth requirements;

6. That we execute the same freely and voluntarily and fully aware of any liability should
the forgoing be proven false or perjurious.
